The Dosing Calculator offers physicians a convenient tool to accurately calculate the equianalgesic dose of EXALGO (hydromorphone HCI) Extended-Release Tablets, CII) from the patient’s current opioid(s). Since some patients are on a regimen of more than 1 opioid, the dosing calculator allows you to enter up to 2 opioids to calculate the EXALGO equivalent. The conversion ratios are based on the guidelines in the EXALGO prescribing information and are only to be used for the conversion from current oral opioid therapy to EXALGO. This application has been certified by an independent third party to match published conversion factors found in the EXALGO prescribing information.
